IN20-212 Prevalence of Mild Cognitive Impairment in Chronic Obstructive Pulmonary Diseases (COPD) at Siriraj Hospital

Abstract

          Chronic obstructive pulmonary disease (COPD) is systemic inflammation that is caused comorbidities such as metabolic syndrome (MetS), type 2 diabetes mellitus (type 2 DM), and cardiovascular disease. Mild cognitive impairment (MCI) has been reported as comorbidities in COPD. However, prevalence of MCI and risk factors involved MCI in COPD, especially male has been minimally reported. Almost prevalence of COPD in Thailand has found in male. Objectives: to investigate the prevalence of MCI and characteristics of MCI in male COPD. Methodology: one hundred and sixty-nine males with COPD recruited into this study between 2017-2019. They were tested the cognitive impairment using Thai version of Montreal Cognitive Assessment (MoCA). The cut point for normal cognitive function when the score is ³ 25 points. The prevalence of male with COPD was calculated. In addition, the characteristics of MCI in male COPD was analyzed. Result and Discussion: the prevalence of MCI in male with COPD was 77.51%. Total score of MoCA of COPD with and without MCI were 18.60±4.34 and 26.21±1.49, respectively (p=0.01). Domains of memory, visuospatial/executive, attention, language and abstract were decreased in COPD with MCI compared with non-MCI, p<0.05. Risk factors of MCI in male COPD might be involved with age, education, smoking, and exacerbation. Conclusions: cognitive impairment was one comorbidities of male COPD and type of MCI in male COPD was the amnestic MCI multiple domains.

Do you observed about the average age of Thai patients that get MCI and how about the average age of your patients?

Thank you for your question. Recently, observation study found that mean age of Thai MCI was 68 yrs. and their prevalence MCI was 71%. This mean age of Thai elderly with MCI was lower than Thai COPD with MCI (age 73 yrs). So, highly age of our patients may be risk factor of MCI in COPD followed with you see the MCI prevalent in this study (77%). However, COPD normaly is a disease in elderly population. it has found since 45 yrs. and hihly peak of prevalence is 60 yrs. So, incrusion criteria in this study was patient with COPD who aged more 45 yrs.

   However, age was not only risk factor to increase MCI prevalent in COPD. Smoking and frequent exacerbation were important risk factors to stimulat a progressive of MCI development in COPD.
