Abstract

     Acne vulgaris is a self-limited disorder of the pilosebaceous unit that is seen primarily in adolescents. Most cases of acne present with comedones, papules, pustules, and nodules with varying extent and severity. There were some reviews about an association between Vitamin D deficiency and acne vulgaris. However, Vitamin D levels in association with acne have not properly summarized yet. This study aims to determine to the relationship between vitamin D level and acne. Methodology: A systematic review was conducted by using the Pubmed, EMBASE, and Cochrane Library, Scopus, CINAHL, and Google Scholar databases up to 31 December 2019 by using the following keywords: (acne OR acne vulgaris) AND (vitamin D OR vitamin D2 OR vitamin D3 OR 25(OH)D OR Cholecalciferol OR Ergocalciferol) The search terms related to design type would be based on the Cochrane Highly Sensitive Search Strategy. Result: Total of ten articles with a total of 741 patients with acne and 652 healthy controls (HCs) were included. Overall, this review shows a significantly lower level of 25(OH)D in the acne group in most studies. Conclusions: This systematic review demonstrated that circulating 25(OH)D levels are lower in patients with acne than in HCs. Thus, our systematic review suggests that 25(OH)D may play an important role in the pathogenesis of acne.

1. there are 2 RCTs (Lim,2016 and Mohamed, 2020) conducted to evaluate the efficacy of vitamin D supplements in treatment of acne, the results shows vitamin D supplements lower the inflammatory lesion of acne and lower acne GAGs score of acne severity significantly in both studies. Study from Mohamed et al. Also shows significantly lower of inflammatory cytokines(IL6, TNF alpha) after getting vitamin D supplements.

 

2. In addition to this study that support the evidence of vitamin D level were lower in acne group than HCs and there are few RCTs that support the efficacy of vitamin D supplements of acne. we might have enough evidence to support the measurement of Vitamin D level before the treatment of acne.

There are many evidence to support the function of vitamin D in the treatment of acne as follows 

1. Vitamin D play essential roles in the skin, such as; Skin proliferation and differentiation, innate immunity in the skin, adaptive immunity in the skin, antimicrobial effects in the skin, wound healing, sebaceous gland.

2. There are many meta-analysis support that Vitamin D  deficiency are more common in atopic dermatitis, psoriasis, vitiligo, and skin cancer.

 

3. There are many literature support the function of vitamin D in Acne 

-Cutibacterium acnes is a potent inducer of Th17, and that 1,25OH2D inhibits c. acnes-induced Th17 differentiation(J invest Dermatology, 2014)

-Sebocytes were identified as 1,25OH2D responsive target cells, indicating that vitamin D analogs may be effective in the treatment of acne.(Indian J Dermatol, 2013)

From existing 2 RCTs used different form of D3 in different dosage and duration as follows 

1. Lim et al, given oral Cholecalciferol 1000 IU/day for 2 months 

2. Mohamed et al, given oral Alfacalcidol 0.25 mcg/day for 3 months
